Included for ALL members:
20% discount on ALL treatment not otherwise noted
$500 off complex/comprehensive clear aligner orthodontic treatment
$375 off moderate clear aligner orthodontic treatment
$215 off mild clear aligner orthodontic treatment
The following limitations to the Des Moines River Dental Care Membership Plan will apply:
Membership savings do not apply to any other locations or specialty practices.
Members cannot be enrolled in a dental insurance plan.
Membership savings cannot be combined with any other offer or courtesy.
Products (toothpaste, bleach, etc.) are not eligible for savings.
When using Cherry Finance or other third party financing, the treatment discount will be 15% off instead of 20% off.
Memberships are active for 12 months, are non-transferrable, and non-refundable.
For all new patients, there will be an additional $150 initial visit fee. New patients will receive a comprehensive exam at the first visit in place of one (1) periodic exam. A full mouth xray (panoramic or FMX) will also be taken at the initial visit.
NOTICE. This direct health care agreement is not health insurance and is not a plan that provides health coverage for purposes of any federal mandates. This direct health care agreement only covers the health care services described in this agreement. It is recommended that you obtain health insurance to cover health care services not covered under this direct health care agreement. You are personally responsible for the payment of any additional health care expenses you may incur.
